The use of the NEURO SWING system ankle joint in vascular diseases

FIOR & GENTZ develops and distributes system parts for manufacturing neuro-orthoses used in patients with neurological gait disorders. In addition to developing innovative products, another goal is to discover new fields of application for neuro-orthoses and the system joints used in them, such as the NEURO SWING system ankle joint. A new area of application is the use of the NEURO SWING system ankle joint in patients with partial foot amputations or vascular diseases (e. g. diabetic foot syndrome and PAD). Such a dynamic system ankle joint fulfills two basic requirements of these patients:
1) Compensation for paralysis or instabilities by restoring the functional forefoot lever: Gait disorders caused by underlying neurological and vascular diseases often have a common cause: the muscles are unable to generate joint-stabilizing moments in the knee and ankle. At the ankle level, the forefoot lever has a functional influence on safe standing and physiological walking. Since the patient is unable to activate the physiological forefoot lever at their disposal, that activation must be replaced by the mechanical forefoot lever provided by the orthosis. This is also applicable if the patient has lost both muscular and bony structures for the anatomical forefoot lever due to a partial foot amputation.
2) Protection of the foot and residual by minimizing plantar foot pressure in case of vascular disorders and/or partial foot amputation: The provision of medical aids for circulatory disorders aims to distribute pressure peaks in the foot area in order to prevent necrosis and tissue death. Own studies have shown that, unlike soft, customized insoles, an AFO with the NEURO SWING system ankle joint does not distribute plantar foot pressure but rather minimizes it. They are therefore ideal to use in combination with such insoles for patients with vascular disorders like diabetic foot syndrome or PAD.
Both requirements can only be fulfilled if the orthosis has pre-compressed spring units, as in the NEURO SWING system ankle joint, and if the orthosis shell and the foot piece are built completely rigid.
